SMS Texting Services Terms and Conditions
This SMS Texting Services provision applies when You have subscribed and expressly consented to receive text messages from Us. By subscribing and providing such consent, You grant Us express permission to send automated text messages to the enrolled mobile phone number(s) through Your wireless carrier unless and until such permission is revoked in accordance with these Terms and Conditions.
When You opt in to SMS Texting Services, We will send You a text message confirming Your enrollment.
Purpose of SMS Texting Services
We offer SMS Texting Services to provide information and resources including, but not limited to:

- Appointment reminders
- Intake communications
- Billing and payment notifications

Message Frequency
Text message frequency will vary depending on the texting program in which You are enrolled.
Help & Opt-Out
As part of enrollment in SMS Texting Services:
- Text "HELP" to receive assistance.
- Text "STOP" at any time to unsubscribe.
After You send "STOP," We will send a confirmation message acknowledging Your unsubscribe request. After that confirmation, You will no longer receive text messages from Us.
If You wish to re-enroll, You must sign up again as You did initially.
Text messages may be sent using an automatic telephone dialing system to the mobile phone number You provided.
Accuracy of Information
Operational information is accurate only at the time the text message is sent and may change without notice.
Age Requirements
Age restrictions apply to SMS Texting Services.
- Participants must be 18 years of age or older and own and control the mobile phone number provided.
- Individuals 14 to 18 years of age may participate only with parental consent.
- Individuals under 14 years of age, or those between 14 and 18 years of age without parental consent, must not participate and must unsubscribe from SMS Texting Services.
Mobile Number Changes
You must notify Us immediately if Your mobile phone number changes. We are not responsible for communications sent to a mobile number that has changed and was not reported to Us.
For Your security, We recommend using password-protected mobile devices and enabling device encryption whenever available.
Marketing Messages & Privacy
Depending on the SMS Texting Service in which You are enrolled, You may receive marketing messages.
We are committed to protecting Your privacy and maintaining business practices that safeguard the security of Your personal information and data.
Discontinuing Service
If You are dissatisfied with the SMS Texting Services or these Terms and Conditions, Your sole and exclusive remedy is to discontinue enrollment in the SMS Texting Services.
Delivery Disclaimer
We do not guarantee successful delivery of text messages. Messages may not be delivered if:
- Your mobile device is outside a service area.
- Network capacity is unavailable.
- Wireless carrier limitations exist.
- Other factors beyond the control of wireless carriers interfere with delivery, including equipment, terrain, buildings, foliage, or weather.
We are not liable for any losses or damages arising from:
- Non-delivery, delayed delivery, or misdirected delivery of text messages.
- Inaccurate or incomplete message content.
- Any use of or reliance upon the contents of text messages.

Content may not be available through all wireless carriers. Participating carriers may include, but are not limited to:
AT&T, Verizon Wireless, Sprint, T-Mobile, MetroPCS, U.S. Cellular, Alltel, Boost Mobile, Nextel, Virgin Mobile, Alaska Communications Systems (ACS), Appalachian Wireless (EKN), Bluegrass Cellular, Cellular One of East Central Illinois (ECIT), Cellular One of Northeast Pennsylvania, Cincinnati Bell Wireless, Cricket, Coral Wireless (Mobi PCS), Cox, Cross, Element Mobile (Flat Wireless), Epic Touch (Elkhart Telephone), GCI, Golden State, Hawkeye (Chat Mobility), Hawkeye (Northwest Missouri), Illinois Valley Cellular, Inland Cellular, Iowa Wireless (iWireless), Keystone Wireless (Immix Wireless/PC Man), Mosaic (Consolidated or CTC Telecom), Nex-Tech Wireless, NTelos, Panhandle Communications, Pioneer, Plateau (Texas RSA 3 Ltd), Revol, RINA, Simmetry (TMP Corporation), Thumb Cellular, Union Wireless, United Wireless, Viaero Wireless, and West Central (WCC or 5 Star Wireless).
Carriers are not liable for delayed or undelivered messages.
Message & Data Rates
Message and data rates may apply to messages sent to You from Us and to messages sent by You to Us.
If You have questions regarding Your text messaging or data plan, please contact Your wireless service provider.
